How our service can help you
The community Parkinsons specialist service provides nurse-led care for people over the age of 18 who have been diagnosed with Parkinsons or Parkinsonism. The service can offer:
- Advice, support and management of symptoms
- Regular clinical consultations by telephone, at a local clinic or home visits
- Proactive group sessions to promote healthy lifestyles including exercise and information/education sessions to help patients manage their condition
- Integrated care to ensure the service is coordinated alongside other services and therapies to improve the management of the condition
- Advice, support and training for health professionals involved in managing the condition.
The specialist nurses are important in keeping people in the community by providing crisis management to avoid unnecessary emergency admission to hospital, reducing length of stay if they are admitted and preventing unnecessary outpatient appointments. It also aims to support people with appropriate medication regimes.
The service also provides GPs with support and advice in dealing with the management of complex patients by providing direct access to minimise GP contacts as appropriate. The specialist nurses also often act as the lead professional for patients with Parkinsons.
