Who we are
The Older People Adults and Community directorate in Cambridge and Peterborough Foundation NHS Trust is a dynamic, vibrant and diverse place to work. We employ a huge variety of professions to work within our services in the community. We deliver these services in our inpatient wards, in people’s homes and in our many clinic bases. No two days in the community are ever the same, and this means that staff often work with different health and social care professionals , including their GP and primary care staff, to provide the best care for our patients.
What do we do in the Older People Adult and Community directorates?
- We employ over 1,600 staff
- We deliver in excess of 800, 000 clinical contacts annually, and have an active caseload of 50 000 patients at any one time!
- We employ over 400 nurses working in a wide variety of services, including community hospital wards, district nursing, and mental health nursing. Nurses are engaged in urgent care within our minor injury units, and rapid response services too.
- We employ specialist services nurses working in respiratory, cardiac, diabetes, epilepsy, stroke, multiple sclerosis, tissue viability and bladder and bowel pathways.
- We employ 600 healthcare support workers.
- We have a wide range of Allied Health Professionals, including 26 Podiatrists, 18 Dieticians, 14 Paramedics, 20 Speech and Language Therapists, 70 Physiotherapists, and 85 Occupational Therapists.
- We also have Psychologists, Psychiatrists, Geriatricians and Social Workers working in our services.
- We employ over 200 fantastic administrative staff who offer critical support for our clinical services, and we engage with many volunteers.
- Last year our staff administered flu vaccinations to over 3500 housebound patients.
- During the pandemic, our staff gave 7,500 housebound patients their Covid jabs. We also looked after people with Covid in our inpatient beds and in people’s own homes.
- We actively encourage staff development, and have a strong track record on developing our staff, enabling career progression within the Trust and wider NHS. We have nurses working in non-clinical environments in management and corporate roles, such as quality and safety teams. We support staff career development and this is shown by many employees starting as Healthcare Assistants or Therapy assistants moving on to train and become experienced clinicians with CPFT.

Neuro Conditions
CPFT Neuro Conditions service comprises 3 specialists, adult, community services; namely Epilepsy, Parkinson’s and ME/Chronic Fatigue Syndrome. Each team offers specialist care, medication management, interventions, advice, and support to patients with these long-term conditions. The clinicians who work in our services are dynamic and highly specialised and they work innovatively and autonomously. A full and varied package of training is provided for new staff, along with regular supervision and support.
Bladder & Bowel Service and Tissue Viability Service
Bladder & Bowel Service and Tissue Viability Service
The core function of the Tissue Viability Service is to provide expert knowledge and skills in solving challenges in skin health and wound healing. Our aim is to meet the needs of the patient by implementing up to date, evidence-based practice, avoiding hospital admission or reduce length of stay.
Prevention of pressure ulcers and lower limb wound management is a key focus of the team. Guiding other health professionals to implementing effective prevention plans to minimize skin integrity problems is a priority, leading to improved patient engagement and quality of life outcomes.
The Bladder & Bowel team provides expert, effective management plans to patients with complex bladder & bowel conditions across the trust. Comprehensive assessment aims to promote respect and dignity by focusing initially on lifestyle interventions and self- management plans.
Equipping other health care professionals with the knowledge and skills in effective symptom management, initial and ongoing care is an integral part of the service.
Urinary & faecal specialist treatment are administered in the community reducing non-elective hospital admissions. Individualized management strategies aim to prevent isolation and help the patient to lead a normal life.

We have 4 Admin Hubs within Cambridgeshire and offer Admin roles from B2 to B7. Along side the Admin Hubs we also have Directorate PA support and Project Officers. We are passionate about supporting our staff to achieve, develop and progress personally and professionally.

Most of us will have experienced a fall at some point. Having a fall will be nothing more than a bit embarrassing for many, however, a fall can be startling, upsetting and life changing, especially as we get older. Falls aren't an inevitable part of getting older, but can become more common and be harder to recover from.
Falls prevention is about identifying and managing the risks related to older people’s health, wellbeing and environment.
The Falls Prevention team in CPFT consists of staff from nursing, therapy, clinical exercise and assistant backgrounds. We are passionate about improving outcomes for older people at risk of falling.
We deliver training and support to CPFT staff and staff from other agencies, to implement evidence-based falls prevention and management strategies. Training and support are offered to those delivering both clinical assessments and evidence-based strength and balance training.
Our Assistant Practitioners support the delivery of multi-factorial falls risk assessments for older people at risk of falling and will visit patients in their own homes. Following an assessment, the Assistant Practitioner and patient will discuss and agree actions to reduced their risk of falling.
This is a specialist service with opportunities to build links with a variety of other services.

A nurse led service that provides individualised person centred care.
The service provides Stages 1-6 of cardiac rehabilitation
Working as part of an integrated team within primary and secondary care the service facilitates the patient’s progress of rehabilitation, ensuring the transition from one setting to another is conducted in a coordinated, efficient and appropriate manner.
The principle aims of the community based heart failure service is to provide a greater level of care in the community – as exposed to secondary care for adults with a confirmed diagnosis of left sided ventricular dysfunction (LVSD) or HFpEF who are registered with a Peterborough or East Cambs and Fenland GP. Therefore, by working in collaboration with primary, secondary and tertiary care the service will support patients with diagnosed Heart failure disease and to promote the wellness, resilience and independence of patients and their families
Diabetes Specialist Team
Diabetes Specialist Team
The service is designed to support people with diabetes, health care professionals and other agencies involved in their care to facilitate self-management and timely intervention to prevent/ delay diabetes related complications, reduce frequency or length of stay of hospital admission. To ensure that the person with diabetes is seen in the most appropriate place, at the most appropriate time by the most appropriate person. We offer the opportunity to undertake developmental posts giving you the experience to work to a higher level, full training and mentoring given throughout. Staff are required to work within a GP surgery as well as a clinic patient setting and in the patients home.

CPFT have four acute admission wards which provide facilities for the assessment and
treatment of older people (younger people may also be admitted on a needs led basis) with
acute mental illness.
The wards are located at Fulbourn Hospital, Cambridge and the Edith Cavell, Healthcare

The memory assessment services (memory clinics) exist in each of the four geographical areas (localities). The memory clinics model aims to offer high quality, high throughput, low cost model of assessment of memory problems to ensure that accurate diagnoses are provided in a timely manner, and an earlier stage in the illness, allowing patients themselves to make decisions about both their current and future care plans.
The Older People's Stepped Care Therapies Service will provide psychological therapy, occupational and psychosocial therapies for people over the age of 65 in Cambridgeshire and Peterborough. Staff will also offer advice, consultation, supervision, training and support in areas of their therapeutic expertise to staff within mental health and integrated older people’s services including inpatient units.

Cambridgeshire and Peterborough NHS Foundation Trust (CPFT) provides integrated community and mental health services of the highest standard due to our continued commitment to quality and service improvement.
As a community dietetic team we aim to provide an efficient and effective service to improve the nutritional outcomes of our patients in various settings and in collaboration with other care professionals.
We are an experienced team with very good support in place for each member. We cover the areas of Diabetes, Nutrition Support, Gastroenterology and Mental Health. We work in a variety of settings including GP practices, health centres, patient's own homes, community hospitals, care homes and virtually.
Our team consists of 26 dietitians and 9 dietetic assistants with good admin support. There are excellent opportunities for development and we provide training for student dietitians.

Intermediate Care Inpatient beds
Intermediate care covers a spectrum of health and social care provision characterised by multidisciplinary team working and a flexible but time limited service. The services operate both at the interface between health and social care and between the primary, community and secondary health care sectors, bringing together a number of services designed to support patients with health needs through periods of transition.
Intermediate Care Community
CPFT provides intermediate care services into people’s homes across the county. Supporting people when they are discharged from hospital to regain their independence in their home environment. The intermediate care teams have Care coordinators, Integrated care workers and Occupational Therapists and Physiotherapists working as part of a team.
Neighbourhood Teams
Neighbourhood Teams
Neighbourhood Teams (NTs) are responsible for the promotion of health and independence in the community working with colleagues across multiple health, social and voluntary sector organisations. NTs offer multi-disciplinary, seamless care closer to a patient’s home. They provide a service for patients in a variety of settings primarily the person’s own home but delivering some interventions in other locations such as GP practices.
Speech & Language Therapy
Speech & Language Therapy
We are a small, friendly and forward thinking team of therapists working across Cambridgeshire. There are opportunities to work in out patient, inpatient and domiciliary (including care home) settings with a wide variety of patients. We see patients with a wide range of conditions including stroke, progressive neurological disorders, respiratory, voice and dementia. Patients are triaged centrally and seen for appointments either face to face or remotely (using video technology). There are regular team meetings, and supervision and peer support is prioritised within the team . We have a small team of clinical assistants and good relationships with our acute colleagues. We encourage clinical development within the team and are also able to access learning and development opportunities within the Trust.

The Community Respiratory Team is a countywide service which provides expert supportive care for respiratory patients, by helping to prevent avoidable hospital attendances, providing facilitated support following hospital admission and the promotion of focused self-management, which will include reviewing those with home oxygen therapy. We are a small, supportive, friendly and forward-thinking team who work autonomously. There are many development opportunities within the team, and we support staff with Advanced Clinical Assessment training and Non-Medical Prescribing. Currently the team is currently involved with many collaborative developments to improve patient pathways for respiratory patients
Podiatry
Podiatry
Podiatry is the profession dedicated to the diagnosis and management of conditions affecting the foot and lower limb.
Within the service there is ample opportunity for professional growth and development across the variety of specialisms we offer; from diabetic and high risk foot management, in both the community and acute settings, to assisting in our community-run Podiatric Surgery unit. Alongside this there is also opportunity to specialise within our MSK or Nail Surgery teams. There are opportunities to develop line management and mentoring skills through supporting of Podiatry students, our apprentice Podiatrists and Footcare Assistants.

The Neuro Rehabilitation Teams aim to work alongside adults with long-term neurological conditions including those with acquired brain injury (including stroke) and progressive conditions (e.g. MS, MND) to help them recover/adjust to episodes of ill health such as stroke
The Neuro Rehabilitation Team provides assessment, therapeutic interventions and management strategies to patients with long-term neurological conditions.
Multiple Sclerosis Specialist Nursing Team
The Multiple Sclerosis Services(MS) provides support patients with diagnosed MS and to promote the wellness, resilience and independence of patients and their families.

This service is a therapy based service who sees people suffering from long covid symptoms and assesses their needs and refers onto consultant led clinics if necessary and provides a holistic therapy programme
