Trafford Ward is a 16-bed inpatient rehabilitation and three-bed end-of-life care ward located at North Cambridgeshire Hospital in Wisbech where patients are treated with dignity and respect and can expect to receive from Nurse with a patient staff high-quality patient-focused care in a friendly environment.

Trafford ward provides a community inpatient service, which avoids unnecessary acute hospital admissions and supports facilitated transfer from acute hospitals. On admission, the multi-disciplinary team include social services, occupational therapists, physiotherapist, nurses and a consultant. They assess patients, identify treatment goals and agree a provisional length of stay. These goals are then reviewed and progress charted at the weekly multi-disciplinary meeting.

Our end-of-life care patients and their families are supported and cared for by ward nurses, specialist nurses and consultant, who will assess, plan and implement care on an individual basis.

Our rehabilitation aim is to help patients achieve a level of independence prior to discharge, which is achieved by our team of doctors and nurses, physiotherapist, and occupational therapist, who work with the patient to help them achieve realistic goals.

The multi-disciplinary ward team undertakes a weekly ward round, and hold multi-disciplinary meetings to identify agree and review patients' therapy and health goals. Day-to-day medical cover is provided Monday to Friday between 2pm and 5pm, by local GPs. The medical team is complemented by a consultant geriatrician.

Discharge planning starts on admission so that complex issues can be addressed in a timely way. Patients and their families are encouraged to take an active part in planning and working together to achieve the best outcome possible in each individual circumstance. If necessary, referrals are made to other agencies/professionals to support patients on discharge.
