Intensive Support Team (Learning Disability)

How our service can help you

The service works alongside other adult mental health teams to support people with a learning disability to remain in their community at a time of crisis, by offering additional mental health support, therapy and treatment. This may include the following:

  • Assessment and formulation
  • Risk assessment and development of risk management plans
  • Develop SAFE plans
  • Medication review
  • Care plan review and development – underpinned by relevant NICE Guidance and Positive Behavioural Support (PBS) approaches
  • Coaching in, and modelling of, implementation of care plan interventions
  • Family/carer support
  • Provider training, education and support using a Positive Behaviour Support (PBS) approach
  • Signposting to other services.

Referral Information

  • The service accepts new referrals from the First Response Service (FRS), Liaison Psychiatry, Crisis Resolution Home Treatment Team (CRHTT) the Hollies In-patient Unit, learning disability, psychiatry, psychology and professionals within the local community team.

  • Parking is limited, so public transport is preferred.

Is there access and support for patients and visitors with a disability?

  • Yes, our clinical bases are accessible. Please ring the team before your appointment to discuss any requirements.

Other essential information

  • We may ask about staffing and current support in place, this is so that we can assess the level of risk. The team comprises several nurses and healthcare support workers, and you may see a combination of staff initially, until a team member is allocated. Our aim is to prevent admission to hospital and placement breakdown. We spend a lot of time in services, so that we can observe and understand the environment and staff perception of service users presentation. Within the visit IST may carry out observations, talk to service users / staff / family, ask to see care and support plans. We look at MAR charts so that we can understand what medication a service user is taking, PRN usage and discuss any physical health conditions. We may discuss the medication with the psychiatrist involved. We may ask for copies of incident reports and ABC charts (this information could be from daily records) and care plans to enable us to analyse them. The gathering of the above information will support developing a Positive Behaviour Support Plan, to find more positive way to work with the individual. We are not here to criticise or scrutinise individuals practice. We acknowledge that some staff teams can find it intrusive to have professionals involved in an intensive way. When the team becomes involved we will maintain regular often daily contact including telephone calls, visits (which could be for long periods of time), we may join staff meetings. This enables us to offer the appropriate support and advice, and also helps when we are developing and reviewing Positive Behaviour Support Plans.

Carers Information

  • A carer is anyone, including children and adults who looks after a family member, partner or friend who needs help because of their illness, frailty, disability, a mental health problem or an addiction and cannot cope without their support. The care they give is unpaid.
