Community Rehabilitation

How our service can help you

We provide comprehensive community rehabilitation and specialist therapy services to meet the rehabilitation needs of people over the age of 18 years old. 

Occupational therapists and physiotherapists work with patients to undertake assessments and develop shared goals and rehabilitation and treatment plans to enable people to fulfil their potential. The benefits can include: Physio exercise with a patient

Therapists are based in integrated teams across four localities: Ely and the Fens, Cambridge City and South Cambs, Huntingdonshire, and Peterborough. They provide a flexible service in a range of settings including inpatient services, intermediate care services, outpatient clinic, day rehabilitation and in the patients own home. Treatment can include:

  • Enabling programmes to increase confidence and independence
  • Exercise programmes (for individuals and groups) to increase mobility, strength and independence
  • Specialist programmes (for individuals and groups) to improve, regain or maintain ability and education to learn about their condition and improve self-management

In addition, therapists are also involved in assessing and ensuring the provision of equipment such as mobility aids or assistive equipment to support activities of daily living. Occupational therapists also undertake assessments for the provision of adaptations to peoples homes.

Referral Information

  • Community Physiotherapy provided by CPFT

    A referral can be made to the service by any registered / recognised health professional. Those internal to CPFT can do so via an electronic referral on SystmOne. Those external to CPFT can do so via e-mail: communityrehabservice@cpft.nhs.uk using the following referral form : https://www.cpft.nhs.uk/download.cfm?ver=6539 This applies to both Cambridgeshire and Peterborough.

    Community Occupational Therapy provided by CPFT

    In Cambridgeshire, this is an integrated health and social care service. A referral can be made to the service directly by the person or by family/carers on behalf of the person seeking help via the Cambridgeshire County Council Contact Centre on 0345 045 5202.     If the person is unable to self-refer and does not have family/carers to refer on their behalf a referral can be made by any registered / recognised health professional. Those internal to CPFT can do so via an electronic referral on SystmOne.  Those external to CPFT can do so via email: communityrehabservice@cpft.nhs.uk using the following referral form : https://www.cpft.nhs.uk/download.cfm?ver=6539

    In Peterborough, for patients who pay their council tax to Cambridgeshire County Council, referrals can be made to CPFT OT in the same way as above for all occupational therapy needs. For people who pay their council tax to Peterborough City Council and who have long-term social care needs (two or more Care Act needs) these will be handed over to the Occupational Therapy Service in Peterborough City Council, whose direct number is 01733 747474.

Other essential information

  • You must meet national criteria to be eligible for a major adaptation to your home. The criteria is to have a substantial and permanent disability as defined by the Housing and Regeneration Act 1996. A permanent disability is one that has endured for more than six months and will not improve with medical or surgical interventions nor respond to rehabilitation.)These adaptations (for example, a level access shower) must be paid for, but can be funded through Disabled Facilities Grants and as such are means tested. Leaflets about Disabled Facilities Grants are available on the Government website. In Cambridgeshire people can refer themselves by calling Cambridgeshire Direct on 0345 045 5202, careinfo@cambridgeshire.gov.uk. In Peterborough people can refer themselves by calling Peterborough Direct on 01733 747474. If you have any problems with the equipment provided or no longer require the equipment please contact NRS on 0330 1000 098.

Carers Information

  • A carer is anyone, including children and adults who looks after a family member, partner or friend who needs help because of their illness, frailty, disability, a mental health problem or an addiction and cannot cope without their support. The care they give is unpaid.
