How our service can help you
The Joint Emergency Team (JET) is an urgent two- or four-hour response service that supports people over the age of 65 or those with long-term conditions in their home environment when they become very unwell and need urgent care, but do not need to go to hospital.
The team will carry out an initial assessment and develop a care plan in liaison with the patient’s GP or out-of-hours GP services.

Referrals accepted via telephone to the Admin Hub on 0330 726 0077.
Professional referrals only. These may include health care professionals, social workers, paramedics, 999 Clinical Triage, 111 Clinical Triage Care staff, care homes and residential homes, and acute hosptal Trusts.
Service criteria
- Any patient over 18 years old who is house-bound and has a long-term condition, with an onset of acute needs
- Patients registered with Cambridgeshire and Peterborough GP Patients living in their usual place of residence, inclusive of Nursing Homes, Care Homes and Residential Homes.
How we can help
- Falls Reduction in mobility
- Acute exacerbation of chronic health issues such as COPD and Parkinsons disease
- Simple chest infections and UTIs (not requiring IVs currently)
- Acute Wound care (not requiring suturing)
- Urgent basic equipment needs – assessment and provision
- Access to up to 2 weeks intermediate care in patients own home following assessment by JET practitioner.
- Access to step-up beds in community hospitals Work with Mental Health Crisis Teams providing physical assessments ruling out organic causes of Mental Health problems
- Bridging healthcare needs for patients at home in partnership with Discharge to Assess, Pathway 1 (Intermediate Care).
Where we cannot help
- Acute medical emergencies such as CVA, MI, GI Bleed, Acute Abdomens,
- Seizures or head injury on warfarin fractures
- Falls where a referral to the JET service is inappropriate (e.g. head injuries on anticoagulant therapy)
- Intravenous fluids or antibiotics
- Social care crisis (with no health needs)
- Acute mental health needs including drug and alcohol misuse
